Behavioral Health/Substance Abuse Program Risk Assessment Survey Process
It is widely held that an independent,
outside consultant is the best resource to use in accessing program risk. The
performance of a comprehensive risk assessment, by an independent party, in
every functional area of your organization, will provide current benchmarks for
your organization's quality of care, patient safety, and risk exposure. It will identify areas of
quality of care, patient safety, and risk management that need improvement. In addition,
the assessment will provide you with best
practice solutions to resolve your issues and also provide you with available
resource information related to the problems identified. In order to assure
continuity of quality of care, improve patient safety, and appropriately manage
risk exposures, a comprehensive risk assessment
should be performed, at least, every two years.
Service Includes:
- Assessment of the organization's commitment to quality and adherence to best practice standards
- Develop a listing of risk areas that should be considered in any program and a listing of risk areas that are
unique to your programs
- Review methods used to monitor incidents, identify sentinel events, and to monitor and track
quality care, patient safety, and risk exposure
- Review of how policies and procedures are developed, updated, approved, disseminated and how implementation is
controlled
- Review of methodology used in communicating policies to staff
- Review of training requirements,
training courses and syllabi and training attendance records
- Review of current policies and procedures related to:
- Business practices
- General program standards
- Behavioral health core program standards
- Behavioral health specific population designation standards
- Employment and community services
Review of quality, safety, and risk management data and minutes
Review of the results of the latest Joint Commission, CARF, or Medicaid/Medicaid review or survey
Interviews with executive clinical and administrative team for Behavioral Health and Substance Abuse, nurse educator,
direct care nursing staff, social service, program directors, attending staff psychiatrists, ED physician, and pharmacist
Environment of care tour of all areas where service is provided
Review of credentialing and continuing education requirements and review of system of documenting credentials and continuing education units obtained
Review of high risk clinical presentations: suicide, restraint, seclusion, elopement, AMA, aggression, contraband, etc…
Medication management review
Medical records review
The organization is provided with a comprehensive report that includes risk
areas, findings, and recommendations.
